I would say that most expensive drugs from overpriced pharma R&D mostly stay in the States, so the impact of US innovation on the contents of that doctor's bag is questionable. Nobody's perfect, but nobody is ripping you off, either.
What I see here is two approaches to risk management. Should failure be punished by lack of medical care for those with low income, and covered by bailouts for ultra high incomers? The excuse seems to be that if the fat cats were left to fail, everybody would lose safety - but was it there in the first place?

I wonder how much health care quality in European countries benefits from innovations in the United States. Not to say that our health care system is perfect, but ripping off U.S. patients does provide big R&D incentives here.

Main thing not covered: lawyers. Any health care system here would be called the lawyer enrichment act. They will sue for everything, especially any refusal to treat one of the 14 million illegals, a number that will probably double when we get a system so the free loaders can get free stuff.
